Chicago Med begins tonight with Dr. Natalie Manning (Torrey DeVitto) facing the consequences of treating her toddler patient against his parents, Janet (Amanda Quaid) and Kyle Harper’s (Alex Kramer) wishes. They were able to get a court order for the treatment, but there is no excuse for her extremely reckless behavior. Dr. James Lanik (Nate Santana) has tried everything to keep her on a short leash. Sharon Goodwin (S. Epatha Merkerson) reminds Natalie how she placed the hospital up for a major lawsuit. They want the boy, Lucas discharged as soon as possible; Natalie assures her that she is not compromised in any way and is of sound mind. Sharon hopes that is true. Dr. Will Halstead (Nick Gehlfuss) offers to supervise her, but she insists she is fine and doesn’t need anyone’s help.

Meanwhile, Dr. Ethan Choi (Brian Tee) has cleared some more space for April Sexton (Yaya DaCosta) at the condo. She only wants the spare bedroom closets, making him promise that they will move after they get married. Together, they try to plan a date, but can only squeeze in half-hour between things, kissing, only to be interrupted by Dr. Noah Sexton (Roland Buck III) at the door. Noah and April go out for their daily run when they spot several men beating up a woman before they take off in their van; she pleads for them not to call the police as she is illegal. Noah tells April to end the call as he checks over Jacinta Nieves (Andrea Cortes). He wants to get her to the emergency room, but she declines, only to fall unconscious and April calls it in anyways.

Jacinta wakes up, as Noah assures her they did not call the police, but she needs to stay in bed for a while. He asks her what happened with April offering her comfort. Jacinta explains she was going to a friend’s place but she doesn’t want him to see her like this and they agree to remain quiet.

Sharon rushes to the elevator, only to catch Dr. Daniel Charles (Oliver Platt) on the other side of the doors. He continues to tell her that he had no idea that Caroline “CC” Charles (Paula Newsome) was inviting Bert Goodwin (Gregory Alan Williams) over for dinner until the very second that Sharon arrived. She admits that Bert invited her out for drinks but she declined. He is not defending CC’s actions, only asking for her to see that CC’s heart was in the right place. She orders him to tell CC to have no more surprise reunions.

Maggie Lockwood (Marlyne Barrett) calls Daniel, saying a patient asked for him specifically, her name is Claire Witherdale (Sophie Marie White). Her former name is Titus, confessing she hasn’t changed her name over on all the legal papers yet, but she asked for him as she remembered him always being sweet and respectful. She has shortness of breath and pain in the chest. He does tell her that he is a psychiatrist, asking her to change into the gown, but her chest hurts too much. Daniel calls for Dr. Choi to help him immediately.

Phillip Davis (Ian Harding) arrives at the hospital, with flowers and breakfast for Natalie. She thanks him as he reminds her the flowers are for their 6 month anniversary. She doesn’t know how the day is panning out and whether she can go to dinner; he gets it, just asking her to keep him posted. Will and Dr. Crockett Marcel (Dominic Rains) witness the exchange before they are called in for a new patient, David Bridges, who was involved in a car rollover accident. He flew out of the car window due to wearing no seatbelt.

Just as Marcel is barking orders and requesting various tests; nurse Doris (Lorena Diaz) finds a “No Blood” card in his wallet, confirming he is a Jehovah’s Witness. Will says the card is at least 10 years old and was signed by his parents when he was a minor, so they need to wait to hear from them.

Natalie is given Lucas’ labs, finding his fever has spiked and she confirms it is not pneumonia; making her wonder what she is missing and asks the nurse to repeat the labs, thinking he may be septic. She refuses to get a second opinion, saying the calling them is not necessary.

April is cleaning up in the bathroom, when she checks her pregnancy test, seemingly disappointed that she still isn’t pregnant. Noah shows her that Jacinta has been in the hospital before and in fact, was born in Indiana. April wonders why she lied, as Noah thinks she is doing this to avoid the police.

Ethan informs Claire that she had a small heart attack but they want to look closer at the small vessels in the heart. A woman named Melinda arrives, confused that she thought she had the wrong room until Claire says her name and she realizes that Claire was once Titus; Melinda was his ex-wife, Doris apologizing that she only called the most recent contacts in her records. Daniel chases after Melinda, asking if she has a few minutes for a coffee and she agrees.

Natalie is reviewing the computer when Maggie checks on her. Natalie apologizes for not noticing something was going on her as Maggie reminds her she had a lot of her own stuff happening so everything is okay. Natalie promises to fill her in on everything, tearing up she admits she was wrong about Lucas’ diagnosis and she cannot make sense of his symptoms and what if this is just a sinus infection and what if her locking him in the small room was a part of her simply wanting to prove that Will was wrong. Maggie knows she would never put a patient in jeopardy and she needs to follow her own instincts as they have never steered her wrong. She comes up with an idea for more testing.

Mr. and Mrs. Bridges arrive to see their son, David lying in the hospital bed. He suffered major damage to his spleen and is still bleeding internally. They tell the doctors that David was spreading the gospel and had a real gift of getting through to people; the immediately say “no” to any blood as they are Witnesses. Will informs them without this surgery, it is unlikely that David will survive; they offer to pray that Jehovah will sustain and comfort David; they do not pray for miracles.

Noah and April confront Jacinta, who is angry they went through her stuff and fears getting out of there before people find out she is there. They promise not to judge and everything remains confidential. She admits this is part of a gang initiation, causing April and Noah to want her to trust them with what is happening in her life.

Melinda tells Dr. Charles that she couldn’t take being shut out from Titus all the time. Daniel says most people hide who they are because they fear rejection. She doesn’t think this would have been easy but she would have tried to understand; admitting that it is too late now for her to understand, simply asking him to take good care of Claire.

Will can’t handle that Marcel is so accepting of what David’s parents had decided. He says they may not like the decisions but they have to respect it and reminds Will that he may need a refresher on medical ethics.

Meanwhile, little Lucas is having a hard time breathing and begins coughing up blood. Kyle orders them to get the IV out of his son but Maggie reminds the parents that there is a court order for her to treat Lucas. He tries to take his son and Maggie has them physically removed from the room, giving Natalie a brief time to get a scope done before they come back.

Jacinta explains her life to April and Noah, explaining she had nowhere to go after foster care so she went to stay at her cousin, Angel’s place and he was the one who brought her into the gang life. She says she cannot just quit the gang as they will kill her before they let her go. Suddenly Angel and another gang member arrive, she says she is fine and didn’t want to come there. She is ordered to get dressed as they are “bouncing.” Noah wants her to stay, standing up to them. April suggests showing them to the waiting room since they refused to leave. Once they are gone, Jacinta is angry that Noah took her and should have let them finish what they started because if they don’t jump them in, they “sex” the girls in.

Maggie informs Natalie that the police are on their way since Lucas’ parents found out that she started treating him before the court order was issued. She feels this is her last chance and needs to figure out what is wrong with him. She is able to do a scope and they find massive amounts of blood in his airways, her realizing this was never a sinus infection.

In the meantime, Doris gives Will David’s toxicology report. Will rushes off as Choi comes with Claire’s results, showing that she has coronary disease and she needs to go off the hormones, which are making it worse. Daniel wants to be there when he has to inform her. Will shows Marcel the report, saying he doesn’t believe David is a practicing Witness as there is alcohol in his blood, 3 times the legal limit and he has cannabis in his system. He also has a tattoo of the Trinity, something the Witnesses don’t believe in it. Marcel informs him that David was in the hospital not too long ago and had his family come in when he had strep throat; Marcel doesn’t entertain Will, saying he chose to have his family make the call, not Will!

Natalie puts a rush on the testing as CPD arrives to arrest her for unlawful restraint as well as medical assault and battery. She pleads with them, saying Lucas has something far more serious, begging them to wait for the labs to come back before they decide to remove him from the hospital. Sharon says she will have Peter meet her at the precinct, but she is told Natalie is not going to be represented by the hospital as she did this to herself and she can find her own way out of the mess.

The CPD is there to take away the two gang bangers, April is chastised from Noah, saying the cops can protect her better than they can. Noah returns to the room and Jacinta is gone, the nurse believing they had discharged her.

Claire is willing to take the risk as Daniel tells her that the next heart attack may actually kill her. Melinda returns unexpectedly to the room, both women apologizing to each other, comforting each other.

Sharon talks to Janet Harper, asking her if she is 100% sure she wants to sign those papers. Sharon knows she would never do anything to hurt her son and is sorry her devotion was in question and this whole situation has created distrust on both sides causing Janet to admit all she wants is her son to be okay. Sharon doesn’t defend Natalie’s actions but she wants her to trust that she and Dr. Manning have the same goal, only to save Lucas.

Noah returns, furious that April only sees him as a kid she needs to micromanage. Will is on the phone, learning that David hasn’t been to the Kingdom Hall in months and no one has tried to contact him. Doris calls him as David’s stats drop. He informs his parents that David will die if he is not operated on within the next few minutes. He tells them he doesn’t believe they know what their son’s wishes truly are and knows David hasn’t done any pioneering in months and when he crashed he was drunk and high. They don’t want to believe him until he confronts them with David’s tattoo. Will says he no longer shares their beliefs and shouldn’t have to die from it. His father walks out of the room, disappointed as his mother leans in and kisses his forehead, only to follow in her husband’s footsteps, leaving the hospital. Dr. Marcel comes over as Will orders him to do the transfusion and surgery as they no longer need his parent’s consent; they rush off to the OR.

Maggie finds Natalie, informing her the charges were dropped and Lucas has an autoimmune disorder called GPA. His parents have come around and the treatment has started, Maggie hugs her as she tells her that she saved the little boy’s life.

David wakes up, learning he can live without his spleen. He asks for his parents as Will informs him that his parents were there but they left. He is confused, causing Will to tell him that they found out he was no longer a Jehovah’s Witness, they left. He is confused as Will explains that they gave him blood during the surgery, causing David to panic. Dr. Marcel encourages him to stay calm, as David says the drinking and drugs were a mistake as was the tattoo; he was going to have it removed and repent. He shouts, “What have you done to me?” as Will looks on solemnly.

Daniel finds Sharon, asking if she is interested in a quick cocktail but she had a rough day. She stops him, asking how his day was. He feels he just witnessed something remarkable, explaining about Melinda and Claire; giving them food for thought.

Noah comes home with Jacinta by his side, letting her stay there for a couple of days and then they will find a way to get her out of town. She thanks him as he closes the door behind him. Back at the hospital, Lucas is doing well, causing Natalie and Maggie to smile. Natalie says she is going to meet Phillip for drinks and will follow Maggie’s advice to trust her instincts.

Sharon arrives at the bar, meeting with Bert, admitting she wasn’t sure she was going to but they order drinks. Will finds Marcel in the lounge, who offers him a drink as they are off duty. Will reluctantly takes the drink as Marcel doesn’t like to kick someone down when they already did it to themselves. Will confesses he couldn’t allow his parents to make this choice and just sit there. Will finally admits that he did the same thing with Natalie, feeling she still loved him but the truth is she chose Phillip and she came to his car that night to tell him she was engaged to Phillip. Marcel reveals she wasn’t going to show him any ring and explains what happened.

Natalie meets Phillip outside, saying this is about them; on the surface, everything seems perfect but in her gut, this doesn’t feel right and she needs to trust herself. She gives him back the ring and says she cannot marry him. Will walks up to them, informing Natalie that Phillip has been lying to her, slipping the ring on her finger when she was unconscious. Phillip denies it, telling her to look at Will as he is only jealous. Natalie throws up her hands and says it doesn’t matter as it is over. She walks away as Will tells him to stay away from her. Natalie demands he stops too as she doesn’t need him saving her. She begs him to get out of her life too.

THE END!
