The worst storm in a long time is headed to the city. Max tries to prep for horrible conditions and lots of emergencies on the way while Iggy talks his ear off.
Max calls Helen. He needs her in to help. She isn’t happy. She was planning on enjoying a night off. Her boyfriend isn’t happy and he senses a vibe between them that makes him uncomfortable.
An older couple argues over the storm and the woman getting her phone to talk to the kids. Vijay laughs a little at them. It reminds him of him and his wife. Meanwhile, Max learns that there is a pile-up outside down the street from the hospital and the ambulances can’t reach them. Max tells the team they need to pair up and find the people who have called 911 who cannot be reached.
Max heads out to help an older woman who is unresponsive. He asks Helen to be his partner. Vijay comes to see his patient. She has suffered an MI. Max and Helen arrive. Their patient is down on the ground. They cannot find a pulse. They use the paddles and get her pulse back.
Reynolds and Jessup find their patient in the snow. He fell and hit his head. He was out getting glucose for his husband who was a 475 before he left. They panic. Jessup runs there while Reynolds realizes his patient is impaled on a rod of some sort. He yells for Jessup but the storm is too loud.
Max needs more supplies. Helen tells him she will go and be right back. His patient mentions she is a clairvoyant and she knows today is the day she is going to die.
Iggy heads out on the roof to fix the antenna after the kids get anxious and board in the pedi wing. The door shuts and he cannot get back in.
Away at an inn, Lauren is asked to help with a patient in critical condition. She tells them she cannot. Unable to help herself, she heads into the room and helps them.
Reynolds works to figure out how a spike penetrated his patient Hugh who is laying in the snow. Hugh tells him he isn’t cold as the snow comes down.
Max’s clairvoyant patient asks him to play cards as they wait for Helen. She reads the card. He helps people even when he has to sacrifice. She talks about a woman whose needs he has tried to meet. It looks like he is going to lose her. He tells him and his wife will be fine. She tells him she isn’t talking about his wife. Helen arrives.
Reynolds works to remove a screw so he can pull Hugh up with the spike in place. Hugh is in pain… Reynolds tells him to talk about his husband. The screw won’t budge. They need to come up with another plan.
Iggy hears the door open. A teen from Pedi comes out on the roof. He looks like he is getting ready to jump. Iggy yells to him.
Max tries to tell Helen about how great she is but he cannot get out the words. He blames it on chemo brain. He heads out to get telemetry equipment at the hospital while Helen stays and talk to their patient.
On the roof, Avi, the teen, tells Iggy that no-one notices when he leaves the room. Maybe if he was really gone no-one would notice too. Iggy tells him he would notice. They are not alone. They work to keep warm.
Vijay has made it to the home of his patient. The husband sees him through the window and starts to break down. Meanwhile, Randolph works to save Hugh by pulling him off the stake. He gets him to the OR.
Max and Helen’s patient tells them she might have been wrong about all of her predictions. Meanwhile, Iggy and Avi get off the roof after a nurse goes looking for them.
Vijay brings the husband to see this wife. The husband cries as he tells her he is here. Lauren admits to her counselor that while she was giving them instructions on their patient, she took some pills. She didn’t take any but she wanted to.
Max isn’t feeling well as they try and head back. He admits to Helen that she was right. She tells him to lean on her as they make their way back to the hospital.
Vijay confides in Iggy that the night his wife died they were fighting. He is upset that he never got to tell her that he loved her smile not that he was sorry.
Max and Helen sit in the lobby while he rests. She starts to tell him how they need to talk about what is going on with them when the power goes out in the hospital. Seconds later the rest of the city goes black.
