The Resident begins tonight with Conrad Hawkins (Matt Czuchry) walking with his daughter Gigi while his wife Nicolette Nevin (Emily VanCamp) is out of town. He insists he has got this as he is super dad and she should enjoy her spa retreat and hangs up taking the baby off to work. Meanwhile, Randolph Bell (Bruce Greenwood) walks into Kit Voss’ (Jane Leeves) office with coffee as she reveals the hospital is in the black for the first time ever. She feels he should be proud that he is the top earner in the hospital; suddenly they are interrupted by a loud noise, learning their systems have been hacked and they have a ransomware attack.

Devon Pravesh (Manish Dayal) is kissing Leela Devi (Anuja Joshi) in the elevator, both agreeing to meet back at his place later that night; they exit the elevator and are ordered into the ER immediately by Irving Feldman (Tasso Feldman) as there are several patients coming in from an accident at the 5K, they voice their concerns as Dr. AJ Austin (Malcolm-Jamal Warner) was doing the 5 K that day. The ambulance bay is busy and AJ is okay, witnessing what had happened.

Conrad arrives with GiGi but has no time to deal with the transition in daycare, running off to deal with the traumas arriving. AJ talks to Winston Robards (Stephen Wallem) and suddenly the monitors are not working and they cannot access his records. Conrad deals with a 14-year-old patient who reveals her painful belly and as they attempt to do an ultrasound it shows “Sorry, you are going to die.” Randolph and Kit arrive revealing they have been hacked and all ambulances have to be diverted; Conrad asks the father to remain calm and they are taking her to imaging. Kit learns their demands are $5 million.

Kit goes into a board meeting, everyone blaming her as she reveals it has happened to over 200 hospitals so far. She reminds them that their patients must be their focus. Dr. Voss has called in the FBI, but the board says they have to fix this or they are both fired, suddenly the TV turns on and a 1-hour timer begins.

AJ and Conrad tell their 14-year-old patient that the scan looks a lot scarier than it is but she is a hero as the blind man she helped only has a broken arm. Conrad goes to update her dad as AJ continues to comfort her. Conrad encourages her dad to sit down, he explains how his daughter is compassionate, loving, and his whole life, and since she was a year old it has only been the two of them. Conrad can relate as he is a father too; his phone rings and it is Marion, saying Gigi is inconsolable and they could use his help. He rushes over and comes to get her immediately. All the children have their ears covered so he puts on a song and she calms down. He tries to get her to say “Da Da” but nothing yet.

Conrad returns to his patient but imaging is now under ransom as well. She can hear them and panics. AJ says they can get her out of the scan but when they get to her she is unresponsive and hypertensive. Conrad tells them to shock her and she is back. Conrad says they need to figure out what is going on before she crashes again; everyone looking at each other extremely concerned.

Marshall Winthrop (Glenn Morshower) walks into Kit’s office, both feeling they were dealt a bum card. She did get insurance for this but they definitely will not pay the $5 million. She learns they have all the hospital’s financial records but she offers them just over $800,000; they feel insulted and now want $10 million; reminding them if they don’t pay, people die!

Back at the daycare, Conrad is told by Marion that they just got Gigi settled and they prefer he didn’t disrupt her again. He reveals he just saw a 14-year-old almost die and this is really hard; she insists it is hard for every parent. In the meantime, Devon meets with his patient Winston, revealing he has a fractured elbow and they have to reset the bone but they can’t access his records so no pain meds. He says he is a strong farm boy and allows them to reset it. Once that is done, Devon spots an upset Leyla and pulls her away into a private room, she admits she never experienced anything like this, explaining that the girl went into v tech and they have no idea why. It is a total nightmare and he hugs her, trying to reassure her as she wonders what kind of monsters would prey on sick people. She reveals someone did this to her sick uncle and took his entire life savings. He kisses he, thanking her for making him remember something.

The sick girl’s father is praying over her when he spots Conrad. The entire medical team walks in and Kit walks in so Conrad excuses himself after learning there are no heart conditions in Esty’s family. He feels they should transport Esty just to be safe. Esty cries out to her daddy, saying her shoulder hurts, AJ checks another spot and they need to get blood into her immediately. Her pressure is dropping, suddenly her father collapses as Aj says their OR is vulnerable to attack. Esty cries out for her Daddy as Leyla tries to comfort her.

Devon reveals that he contacted someone who may be able to help with the ransomware attack and Winston needs to be moved for his splint; but he suggests he can go on his own with his cane, sure that the elevator has braille. Esty’s father wakes up, AJ and Conrad explaining that he had the same v tech as Esty. He suddenly grabs his chest and is having a heart attack, even though he just explained that he was at Atlantic General and they did an EKG on him and everything was normal. Conrad orders medications and rushes off to call the hospital.

Marshall and Kit demand the board to pay the ransom as they have critical patients that need to be taken care of, they don’t care if they get fired. Devon comes in with his friend, revealing they have a chance to turn the tables on the hackers. He asks them to stall as long as they can. Conrad is on the phone, learning he was right about the syndrome and it was only a slight change in the rhythm. He asks them to email the scans.

The orderly brings Winston down to the fracture clinic, where the nurse has a brutal headache. He leaves him there to wait to be seen. Winston asks him to thank everyone in the ER. Meanwhile, Conrad and AJ realize they have less than an hour to save him, Conrad calls Devon, saying he needs his help. The medical team dealing with Esty realizes she is leaking spinal fluid and needs immediate surgery; Devon calls Conrad and he tells him to talk fast as 2 lives are on the line. They found the power station that will light up one OR. Conrad says they are on their way with 2 patients.

Winston explains that he has been waiting for too long and the nurse says their tech went homesick; she says she will go find someone and will be back as soon as possible; he pleads for her not to be too long.

In the OR, Randolph finishes removing Esty’s spleen and has to help AJ with the heart. Leela takes over as she has a photographic memory from the scan. Conrad is on edge as he watches from the gallery. Conrad rushes into the ER saying they need to keep trying and think about Esty. AJ and Conrad shout at each other but Randolph tells Leela to stop the cardiac massage and call it; feeling this would have happened regardless. Conrad is heartbroken.

Winston is fed up and begins to wander down the hallway, calling out for anybody. He wanders into a room where there is fumes in the air and he falls to the ground choking on the air. Devon and his friend are working on the computer, Kit delaying the hackers. They successfully get the IP address and open the camera of the hackers’ computer and reveal they are not in Russia at all but in Baltimore. Kit tells them to not bother fleeing as the FBI is almost there to greet them; everyone starts clapping in the boardroom.

Leela checks on Esty while AJ and Conrad look on heartbroken. AJ says he is the one always telling people to not get emotionally involved but Esty was a special girl and her father will not see her graduate nor walk her down the aisle. Randolph reminds him they did everything humanly possible to save her father. He says Conrad is the one who saved Esty as Conrad has a social worker looking to find her mother and has hope as it is hard not to love a child like Esty, but AJ is angry that her mother already left her once. Esty reveals that she has an uncle that her father listed as next of kin; Leela says she will call him so he can be there when they tell her.

Conrad leaves to go get Gigi but finds that Marshall, Devon, and Kit have her already, saying it takes a village to raise a child. They reveal how Devon and his friend found the hackers and rescued the hospital; Conrad admits he knows how lucky he is, never more so than in this moment. Devon and Leela kiss in the locker room and he suggests they both go to their respected places and sleep. She suggests in the morning they can talk more about moving in together, he agrees. Devon says good night to Irving and Nolan but notices that Winston was never checked out; Nolan thinks he must have left without checking out but Devon goes to check on his patient.

Once in the basement, Devon calls out, finding no staff around. He finds Winston on the ground, calling out for help beginning to choke on the air as well.

Randolph and Kit walk down the stairs, him suggesting they go out to get some food. She says she is tired, they kiss each other good night and they’ll see each other tomorrow. Conrad goes home places, Gigi, in the playpen, grabbing her a baby bottle and himself a beer. He notices that he missed a call from Nic and messages her good night. He brings the baby to bed, but she holds onto his shirt, saying “MaMa” and he tells her that dada will have to do until tomorrow. She then says, “Da Da” and all he can do is smile and hold her tightly.
